Here’s Some Info To Help…
If an employee (and others in their respective tax household) had health coverage for the entire year (employer sponsored, individual plan, Medicare, Medicaid, etc.,) they’ll only need to simply check the box on line 61 of Form 1040, line 38 of Form 1040-A, or line 11 of Form 1040-EZ and they’re done.
If an employee or a dependent purchased coverage on the Marketplace AND received a subsidy they’ll receive a Form 1095-A, Health Insurance Marketplace Statement from the government showing the amount of subsidy they received in 2014. They’ll need to file a Premium Tax Credit Form –

Three forms you may be asked about (or wondering about yourself):
- Form 1095-A — Issued by the federal government (CMS) to individuals who purchased a health insurance plan on the Marketplace and received and received a subsidy, formally referred to as an advanced premium tax credit (APTC).
- Form 1095-B — Issued by insurers to groups and individuals On/Off Marketplaced who did not receive a subsidy or APTC. The 1095-B Form is NOT REQUIRED for the 2014 tax year. Some carriers will be sending these forms to members, many will not since it isn’t required
- Form 1095-C – Issued by large employers self funding employee coverage. These are for members that obtained insurance through fully self funded employers programs. These forms NOT REQUIRED for 2014’s tax filings.
